Support for the moments that feel overwhelming.

CT Health Advocate helps you understand what's happening, explore your options, and know what to do next.

When you're caring for an aging parent or loved one, there are moments nobody prepares you for.

A hospital stay that leaves you wondering what happens next.

A feeling that something isn't quite right, but you're not sure where to start.

A doctor's appointment where you leave with more questions than answers.

Trying to coordinate care while balancing work, family, and everything else life demands.

— You’re in the right place.

Most families never expected to become a care coordinator, medical record organizer, appointment scheduler, and advocate for someone they love.

You shouldn't have to guess which provider to call, wonder if you're asking the right questions, or try to make sense of everything on your own.

We're here to help you break it down into clear, manageable next steps so you can make decisions with confidence.

Yet that's exactly what happens.

"Dear Ria,

You and your team were so important to me

Helping Dad stay healthy and happy, couldn't have happened without your service, thank you!"

— Love R....

Care Strategy Session

This service is a good fit if you:

• Have questions after a new diagnosis
• Are trying to decide between care options
• Feel overwhelmed and don't know where to start
• Want a clear plan before making decisions

A Clear Plan Forward

An adult daughter wasn't sure what to do next for her father.

The Situation

She knew her father needed help, but she wasn't sure where to begin. She wanted to understand what resources were available, how to talk with his healthcare team, and what steps made the most sense for their family.

How We Helped

During a one-hour Care Strategy Session, we talked through her concerns, answered her questions, identified resources that fit her father's situation, and created a personalized action plan with clear next steps.

What Changed

She left with a clear plan, practical next steps, and the confidence to move forward. As she put it, "That one call gave me clarity and relief."

Transitioning Home

Coming Home With Confidence

One family wanted to bring their loved one home but didn't know where to begin.

The Situation

Their loved one was being discharged from rehabilitation, and they wanted to make sure everything was in place before returning home. They weren't sure what questions to ask or what support would be needed once they got there.

How We Helped

We reviewed the discharge plan, explained what it meant in plain language, identified the services they would need, reviewed their long-term care insurance benefits, and created a personalized 30 day plan so they knew exactly what to do after discharge.

What Changed

Their loved one returned home with the right support in place, and the family knew what to expect, who to call, and what the next steps would be.

This service may be right for you if you:

• You want to feel prepared before your loved one comes home
• Need help understanding the discharge plan
• Want support throughout the transition home

Aging in Place Plan

When staying at home is the goal

One family wanted to honor their father's wish to remain independent while moving him closer to the family.

The Situation

A son wanted his father to live closer to family, but he was concerned about finding new doctors, arranging the right support, and making the transition.

How We Helped

While reviewing his medical records, we discovered a physical therapy referral that had been recommended but never followed up on. We coordinated his transition to new healthcare providers, connected him with the right support services, and created a personalized plan for his move.

What Changed

He successfully moved closer to family, regained enough strength to stop using his walker, and continued living safely at home. His son could spend time being a son instead of trying to manage everything himself.

You may benefit from this service if you:

• Want to help a loved one remain safely at home
• Are noticing changes in memory or mobility
• Want to plan ahead before a crisis happens
• Need an objective assessment of the situation

Local Support

Peace of Mind From Miles Away

Several siblings wanted someone nearby they could trust to help coordinate their mother's care.

The Situation

They were balancing careers, families, and life in different states while trying to stay on top of their mother's healthcare. Every visit or phone call with their mom was spent catching up on appointments, phone calls, and problems that needed to be solved.

How We Helped

We became their local point of contact by attending appointments, communicating with providers, organizing medications, coordinating services, and keeping the family updated so everyone stayed informed.

What Changed

Instead of spending every visit managing healthcare, they could spend time simply being with their mom, knowing someone they trusted was helping keep everything on track.

This service is designed for families who:

• Need ongoing guidance
• Manage multiple healthcare providers
• Live out of state
• Want a trusted healthcare partner

What are the first steps?

What are the first steps?

Most families begin with a complimentary 20-minute Discovery Call. During the call, we'll briefly discuss your situation, answer any questions you have about my services, and determine the most appropriate next step.
If you'd rather dive right into your questions and leave with a clear plan, you can schedule your Clarity Session instead.

What if I'm not the patient?

What if I'm not the patient?

Most of our clients aren't the patient. They're adult children, spouses, or other trusted decision-makers looking for guidance.

It's okay if you're not ready to involve the person you're helping right away. We can talk through your concerns, discuss your options, and create a plan before they're ever involved.
If we move forward with ongoing services, we'll work directly with them whenever possible. If they're unable to make their own healthcare decisions, we'll work with their legal representative while always keeping their wishes, best interests, and dignity at the center of our work.

How are you different from a hospital case manager or discharge planner?

How are you different from a hospital case manager or discharge planner?

Hospital case managers and discharge planners play an important role in helping you navigate care while you're in their setting.

We're an independent, private-pay practice, which means we work directly for you and your family. Our support can go with you before, during, and after a hospital stay, rehabilitation stay, or medical appointment.

Our focus stays on your goals, priorities, and what matters most to you as care changes.

Do you attend medical appointments?

Do you attend medical appointments?

Yes. We can attend appointments in person or virtually to help you prepare questions, understand what's being discussed, and keep track of next steps.

We can also help with follow-up after the appointment when needed.

What services don't you provide?

What services don't you provide?

Our role is to educate, advocate, and help you navigate the healthcare system.

We do not:
- Make medical, legal, or financial decisions on your behalf
- Provide hands-on medical or personal care
- Prescribe medications or provide medical treatment
- Act as your loved one's Power of Attorney, conservator, or healthcare representative
- Provide emergency or crisis services

If your situation requires a service we don't provide, we'll do our best to connect you with the appropriate healthcare professional or community resource.

How do I introduce your services to my loved one?

How do I introduce your services to my loved one?

Many families worry about how their loved one will react, and that's completely normal.
A simple way to introduce us is to make it about supporting you, not taking something away from them.

You might say:
"Ria helps families navigate situations like this every day. I've asked her to help us figure this out so we can make the best decisions together ."

Framing it as support for you, while keeping the focus on their goals, often makes all the difference. If you'd like, we're happy to help you plan that conversation before we get started.

What paperwork will I need to complete?

What paperwork will I need to complete?

The paperwork depends on the service you've chosen.

Care Strategy Session:
- Intake form
- Service agreement

Comprehensive Services:
- Intake form
- Service agreement
- Release of Information so we can communicate with your loved one's healthcare providers and care team
- Patient portal access, when available, so we can review medical records and prepare for our work together

All paperwork is completed electronically before services begin. If additional information is needed, we'll let you know and walk you through the process.

You don’t need to have everything figured out before reaching out.

The initial call is simply a space to talk through what’s happening and explore what support may be helpful.

You don’t have to carry this alone.
